$235.00 Patients eligible for Medicare will not be billed. All others, fee applies. Mon-Thurs collections, Disease status and therapy and completed Referral form (see link) must accompany sample.Send to lab within 24 hrs. Sample must arrive at Frome Rd Lab by 16:00 (same day) if collected on a Friday. GMP staff will address the payment requirements when the patient sample and test request is received by the lab.

Must be received by Sth Aust Pathology  within 24 hours of collection fresh or stabilised in trizol within 24hrs and sent frozen.

Referral - BCR-ABL Kinase Domain Mutation Analysis form (see below) must be filled out by the Haematology Registrars and sent with the pathology request form and sample.

Must be received by SA Pathology (South Australia) within 24 hours of collection when fresh unless stabilized in trizol and frozen.
