For quantitative CMV PCR order CMV Viral Load.

A separate EDTA blood tube is essential. It cannot be used for other tests.

Use a Gene expert (xpert) swab (red top swab). 

Guidelines for swab collection.

A sterile flocked swab is used to vigorously swab the base of the lesion— applying enough pressure to collect epithelial cells without causing bleeding—and collect vesicular fluid. It is important to collect infected epithelial cells from the base of the lesion because they usually contain a significant amount of virus. 
Break off swab into red top swab container.

For investigation of CMV colitis, a colonic biopsy is recommended. For all other enquires, please contact the Microbiology Registrar.

Dedicated EDTA tube. Flow Cytometric analysis of cells stained with a range of antibodies against lymphoid and myeloid markers. The panel of markers used will be appropriate to the clinical notes supplied or as arranged by prior consultation. Samples must be marked "Urgent", must reach the laboratory as soon as possible, and will not normally be accepted after 5pm Monday - Thursday, and on Fridays. All out-of hours testing requires notification to the on call Laboratory Haematologist (via switch) and the laboratory. An FBE must also be requested and collected (additional 3mL EDTA tube)) if Leucocyte Immunophenotyping is requested on peripheral blood.

This is a CSF test. Serum sample must be collected at the same time for interpretation of results. Serum alone is not suitable for this test. Discuss with Biochemistry if only serum (and no CSF) is received.

Investigations for ova, cysts and parasites are restricted to Outpatients and patients hospitalised for less than 4 days.

See separate entry for cryptosporidium or cyclospora stains. These tests must be requested separately.

Only one (1) faecal sample is required. It must be placed in the fixative contained in the faecal collection kit available from the laboratory.

Strongyloides culture

Pathologist approval is required for Strongyloides stercoralis culture. Phone the laboratory for special collection instructions. Specimens must be labelled URGENT and arrive in Microbiology within 1 hour of collection between 0800 and 1630. Do not refrigerate.

Duodenal aspirates must arrive in Microbiology within 1 hour of collection. Mark specimens URGENT. Do not refrigerate.

Enterobius vermicularis (Pinworm)

The specimen should be obtained in the morning before showering or using the toilet. Take about 3 cm of transparent cellulose (sticky tape). Eggs adhere to the cellulose tape and can be detected microscopically in the laboratory.

PArvovirus PCR is only indicated in a small subset of patients. Please discuss the test with the Head of Microbiology if PCR is requested.

If the request slip only asks for Parvovirus, then serology PVS should be coded.

Collection & Request Instructions

Aspirate from collection, abscess or wound; biopsy of abscess wall. The specimen should be collected in a sterile container. It is also possible to send a capped syringe (needle must be removed).

Pus should always be aspirated, if possible, rather than collecting a wound swab.

Swabs from a draining wound or sinus are frequently contaminated with normal flora from skin, mucous membrane.

Pus should be transported immediately to the laboratory.

Biopsy of abscess wall or adjacent tissue may increase the yield, especially in chronic infection.
