$235.00 Patients eligible for Medicare will not be billed. All others, fee applies.

Collection times: Tuesday afternoon (after 2.30pm) to Wednesday morning (before 8.30am) collection. Send to NPV lab immediately (especially with Wed AM collection). Outside of these times, please seek approval from the Lab Haem Reg/Consultant prior to collection.

Disease status and therapy and completed Referral form (see link) must accompany sample. The samples must be couriered to SA Pathology within 24 hrs. GMP staff will address the payment requirements when the patient sample and test request is received by the lab.

Referral - BCR-ABL Kinase Domain Mutation Analysis form (see below) must be filled out by the Haematology Registrars/Consultants and sent with the pathology request form and sample.

Must be received by SA Pathology (South Australia) within 24 hours of collection.
