Registration Instructions:

A request form signed by the Oncologist or Oncology Registrar is mandatory.

This is usually an add-on test to a tissue/histology specimen. If the tissue specimen is not specified on the request form, contact the requesting doctor to confirm the tissue specimen for the test. The test should be registered under the same request number as the tissue/histology specimen.

After registration, e-mail the scanned request form to anatomicalpathology@alfred.org.au, then document in Tracking that the request form has been e-mailed.
